2024-25 School Year New York State Immunization Requirements for School Entrance/Attendance
All children must be age-appropriately immunized to attend school in New York State. The number of doses depends on the schedule recommended by the Advisory Committee on Immunization Practices (ACIP). Intervals between doses of vaccine must be in accordance with the “ACIP-Recommended Child and Adolescent Immunization Schedule.” Doses received before the minimum age or intervals are not valid and do not count toward the number of doses listed below. See footnotes for specific information for each vaccine. Children who are enrolling in grade-less classes must meet the immunization requirements of the grades for which they are age equivalent.
Mandatory Physicals for
Grades 7, 9, and 11.
All physicals are good for 1 year only, this includes sport physicals.
Physicals are mandatory for all students that are entering grades 7, 9 and 11.
Physicals that are still valid ( within 1 year) at the start of that school year, are accepted for entrance to those grades.
Those students entering 7, 9 or 11th grade, whose last physical was over 1 year prior to the start of school year should have a new physical before the start of the new school year.
Please be aware that these are NYS Department of Education regulations.
We ask that you, as parents, be proactive and check to see that your student is in compliance and if not, please take steps to have these done before school starts.
Most medical offices are booking out months, so consider calling and scheduling early.
**New 6th graders who want to play sports - the best time to get your physical is April-June. This will cover a complete year of sports.
NYS Immunization Reminder for
Students Entering Grades 6, 7 & 12
New York State Public Health Law, Section 2164 requires students to be up to date on certain vaccines in order to attend school. All students new to the district and those entering Kindergarten will need to be current on these vaccines. Please be sure your child has all required vaccines prior to the start of school in September to avoid exclusion.
Reminder for parents/guardians whose child will be entering either grades 6, 7 or 12:
· Students entering 6th grade (who are 11 years of age or older) must have one dose of Tdap vaccine. Your child may receive this vaccine as early as 10 years of age.
· Students entering 12th grade must have two doses of the Meningococcal vaccine (or one dose if the first dose was given at age 16 or older).
· Students entering 7th grade must have the first dose of the Meningococcal vaccine.
· There is no exemption for not getting the vaccines
We encourage you to speak to your child's primary care provider to confirm your child is up to date on these vaccine requirements.
Please be aware that the nurses do not work in the summer. However, they will check their emails periodically. We ask that you forward all records to your child's perspective schools via the following emails:

This includes not only prescription medication, but also all over-the-counter medications such as Tylenol®, Caladryl®, cough drops, throat lozenges or spray, antiseptic ointments, etc. The school nurse can only administer these medications if the parents provide a note from the family physician, a note from the parent, and the medication in the original container marked with the student's name.
Vaccination Requirements - Applicable to all Students
Public Health Law §2164, as amended by Chapter 35 of the Laws of 2019 applies to students attending all schools as defined in Public Health Law §2164 to include any public, private or parochial child caring center, day nursery, day care agency, nursery school, kindergarten, elementary, intermediate or secondary schools. Public Health Law §2164, as amended by Chapter 35 of the Laws of 2019 prohibits a school from permitting any child to be admitted to such school, or to attend such school, in excess of 14 days without sufficient evidence that the child has received all age appropriate required vaccinations. The 14 days may be extended where the student is transferring from out of state or from another country and can show a good faith effort to get the necessary evidence or where the parent, guardian or any other person in parental relationship can demonstrate that a child has received the first age-appropriate dose in each immunization series and that they have age appropriate scheduled appointments for follow-up doses to complete the immunization series in accordance with the CDC's Advisory Committee on Immunization Practices Recommended Immunization Schedules for Persons Aged 0 through 18.
